1. Intense or Escalating Pain
It is normal to feel some tenderness or a "stretching" sensation for 24–48 hours. however, throbbing, sharp, or escalating pain that does not respond to over-the-counter pain relief is a major red flag. This can indicate nerve compression or, more seriously, a blockage in blood flow.

2. Skin Blanching or Mottling
This is perhaps the most critical sign. If the skin on or around your lips turns stark white (blanching) or develops a blue-ish, lace-like pattern (livedo reticularis), it may signal a vascular occlusion. This happens when filler accidentally enters a blood vessel, blocking oxygen to the tissue.

3. Abnormal or Asymmetrical Swelling
While some swelling is expected, it should be relatively uniform. If one side of your lip balloons significantly more than the other, or if the swelling extends toward your nose or cheeks rapidly, you may be having an allergic reaction or a localized infection.

4. Heat and Persistent Redness
If your lips feel hot to the touch or look increasingly red several days after the procedure, you might be facing an infection. Healthy healing should see redness fade within the first 48 hours.

5. Visible Lumps or "Migration"
Filler is meant to stay within the vermilion border of the lips. If you notice hard, painful nodules or see the filler "traveling" above the lip line (often called a filler mustache), the product may have been injected too superficially or in excessive quantities.

6. Pustules or Blisters
The appearance of small white bumps, pustules, or herpetic-like blisters is not normal. This could be a sign of a viral breakout triggered by the needle trauma or a bacterial infection that needs antibiotics.

7. Numbness or Loss of Function
While local anesthesia can cause temporary numbness, a prolonged loss of sensation or the inability to move your lips properly after the numbing agent should have worn off indicates potential nerve involvement.
